Distracting the defender (1)

We have a complicated position with both sides knocking on the other side's door, ready to deliver a crushing blow. There's no time for carefully prepared new plans; the ones leading here should be good enough. What it is needed now is the following:
a) Quick analysis to determine where we stand and more important where are the weaknesses on each side
b) Black to move needs to find the best possible line
c) Can Black use "distracting the defender" method? Who's the defender Black needs to distract?

Puzzle #186 solution:
Dus Chotimirsky - Bannik, USSR 1949. Philip and Ziyao have provided the most thoughtful answers again. Here is Philip:
a) White is down by one pawn and is in check. White's d5 Knight is also under attack. But it's White's turn. White's Queen and two Knights are in good positions to attack.
b) White is able to hold the fort against Black's killer diagonals. See solution:
